How to Apply
NC4K is pleased to offer the annual Eden Adams Memorial Scholarship with the intent of benefiting those affected by pediatric cancer by helping to fund their education. This scholarship is awarded to one recipient every academic year in the amount of $1,000. Funds will be paid directly to the accredited institution; one half will be disbursed each semester. The NC4K Scholarship Review Committee has the sole discretion to make decisions regarding the scholarship recipient, and all decisions are final.
To be eligible for the Eden Adams Memorial Scholarship, one must:
Have received a cancer diagnosis before the age of 19.
Live in Ohio or have received treatment for cancer in Ohio.
Be a citizen of the United States and attend school in the U.S.
Be in a registered NC4K Family.
Maintain a record of community service.
Be a high school senior, high school graduate or current post-secondary undergraduate who plans to enroll in full-time undergraduate study at an accredited two- or four-year college, university or vocational-technical school for the entire upcoming academic year.
Have a minimum grade point average of 2.5 on a 4.0 scale (or the equivalent).

An official transcript of academic records from high school or college, whichever is applicable.
An acceptance letter (on letterhead) from the academic institution you will be attending. If already enrolled, please provide a copy of your registration confirmation.
A verification of diagnosis (on letterhead) from your physician.
